From hospital discharge to community life

We accommodate people discharged under Section 117 of the Mental Health Act into our Supported Living — helping them transition back into community living at their own pace, safely and with professional support throughout.

We manage each individual's transition carefully, working alongside a full multi-disciplinary team (MDT) of professionals to ensure coordinated and person-centred care.
